Our skilled California-licensed RNs and LVNs deliver compassionate, professional nursing care to homebound patients with intermittent but ongoing needs, focusing on symptom management, medication administration, wound care, and disease management while providing essential treatment interventions to help patients remain as independent as possible in their own homes; their responsibilities include comprehensive health monitoring and assessment, chronic disease and symptom management, medication reconciliation, management, and education, post-surgery or hospital transition support, chronic health condition care management, lymphedema and wound care management, ostomy care management, as well as infusion, IV antibiotics, and central line care, all while keeping physicians fully informed of patients’ status and progress throughout their time under home health nursing care.
